create form from text file or spreadsheet

Full disclosure, I just initiated a trial of Smartsheets and haven't yet taken the time to learn its features; this might be as easy as copy paste.
Is there a way to create a form from a text file or spreadsheet list? I have a long list of items I want to track during user tests, and thought a form might be a good way to collect that data. Once created, I would want to add relevant inputs for each row such as "time user took to complete task" or a text input for a user suggestion.

Hello Josh,
A form is definitely a good way to collect data. However a form is based on a sheet. The most easy way to have your form based on an existing spreadsheet is to import that spreadsheet first by importing that spreadsheet. Have a look at the screenshot how to do this.
