How do I capture data via a form and link it to a unique identifier I already have on a sheet.

Problem:

We have a list of students with Unique student ID's.

We intend to carry out a training session for students on this sheet and need to record their attendance .

Ideally if i can have a form link made into a qr code that students just scan and enter or select their student ID ( this data is already on my sheet). Once they select their ID they just click submit which should then capture date and time submitted as an attendance timestamp. Also once done the link should not allow them to use it again.

Any help partly of fully to provide a solution would be very welcome

I would suggest having the form attached to a second sheet then using an INDEX/MATCH or VLOOKUP to pull in the time stamp to the record sheet.
