How to Return Many Values to Track Status Changes

We are trying to track the dates that tickets change from status to status. Using workflow, I created a new line added for each status change. But now need to return the dates on each of those lines entered. Below are screen shots of the two sheets I am trying to do this. Tried Vlookup, Match and Index but cant get them to work together correctly.

You will see the highlighted example where I am trying to pull the Open Date and Corrected Date.

We are trying not be introduce another tool/application to the environment. Any other options?

My solution is all in Smartsheet, just using another sheet for the process.
