Sales CRM with INDEX/MATCH and data shuttle

Using Smartsheet as a sales CRM, we have a master sales sheet in Smartsheet with a column for a unique customer ID. Since there are 2000 customers, it's not reasonable to update the sales numbers for each customer manually on a monthly basis. We pull a report from one system in an Excel file that also has the unique ID. I would like to be able to use data shuttle to upload the sales report into Smartsheet using data shuttle (if needed) and use INDEX/MATCH to populate the sales numbers into the sales sheet on a monthly basis by matching the unique ID. I cannot figure out how to do this?

I am having difficulty with writing the expression that will match the ID and then only fill in the month column.

Each of our referring providers has a unique ID. What I would like to do is upload an excel into data shuttle with an expression that will index/match the facility ID on the excel with Smartsheet and then fill in the columns based on the month with the referral received.
