How do I create Form that doesnt add new line?

I want the form to use data to request information that is not available in the spreadsheet.

I have columns for Part, Quanity, Need by date - and I want the requestor to be able to fill in the form which goes to someone to follow -up on - but the follow-up happens outside of smartsheet and I dont need a record of what the request was

Unfortunately all forms will add a row to the sheet it's attached to. The only options you have are whether the rows are added to the top or the bottom of the sheet.
