How to make a form add a new row each time it is completed?

I have a form set up with an automated request every two weeks. However, after the first time a user has filled out the form, the next time they see it they see the data from the row they created last time.
How can I make it so that each time a user completes a form it adds a row to the table? I'm tracking progress updates and want to keep track of previous entries.

Ah ok. Using this method sends them to an existing row, so yes they will see what they entered last time. Are you wanting to have them add a totally NEW row to the sheet through a form?
If so, go to your Sheet, then click the forms link at the top. Create a form. Once you've finished, open the form and copy that URL. You can then drop that URL into your automation.

I have created a form for the sheet, and am sending an update request. It does have a link to the form ("Open request")
