Approval Workflow Issue

Hi All,

I have recently encountered a rather weird issue with an approval request workflow added to one of my sheets. This has never happened before with any such workflows on other sheets.

On one of my sheets, we send out an approval request on items when they reach certain status and the request seems to be getting declined as soon as the user clicks on "Open/View Request" link. I have checked with the users on multiple occasions the steps they take and all seem to be doing the right thing. I am not sure why the requests are being automatically cancelled. Has anyone else noticed this?

i have checked my workflow and it looks correct. Can someone please help?

@Paul Newcome Yes, i have other workflows.

I deleted all the workflows and added them again. Everything was working fine till today - one of the workflows got triggered but the alert was sent to someone who wasn't on the alert.
