Search function in a form that searches the Data in the Smartsheet

New to Smartsheet. I have a form that collects information when we issue PPE to our employees. When the employee enters their employee number, I would like the form to search and display when sand if the employee has been issued PPE prior and when it was issued. This will allow us to request the old PPE (safety vests or hard hats with company logo) to be returned to us for disposal. I don't want the people issuing the PPE to have access to the smartsheet.

We still want to be able to complete the form no matter the data returned. How would I build this? I understand Excel formulas but Smartsheet has little differences that trip me up while I build the formulas. Thank you.

Here's a possible workaround or workarounds

• You could add the form to a dashboard and show a report on one side showing the information for the employee.

I don't know. I'll try creating this on the Dashboard and see if this will create the results I'm looking for.
