Create a Unique Form within Template

Is there a way to link a form within a template that will be associated with the NEW document created by using the template?

CONTEXT:

Template Purpose: each time site is visited by senior team, a template is created for site visit to document walk-through notes.

Form Purpose: individual senior team members would click the form link embedded in document made from template to track walk-through notes

Current Issue: When form linked in the template is clicked in new template-generated document, the completed form info inserts into the template, not the new document.

How can I create a unique form-link that will integrate to the newly generated document from the template so each new doc has it's own form to funnel information?
