Creating an approval form from a submitted user request form

Hi,

I have successfully created a form that allows a new user to provide their information, their requested access level, and their supervisor's name. When the new user submits the form, I want another form to be triggered and sent to the supervisor for their approval for the new user. Thus, the form would include the new user's name, requested access level, and if the supervisor approves. This would allow me to time stamp it and ensure the proper approvals.

I understand that I could allow the supervisor access to my smartsheet to make this work but I would like to keep the supervisor out of the smartsheet itself.
