How to map form fields to columns manually

I am testing the form feature for the first time. I imported a basic excel table then edited the form as needed in the form editor. After filling out my test form and successfully submitting a few times I go to the sheet where answers should populate and there are blanks where there should be data. These blanks are not consistent across test cases (for example, for one test the 'first name' field populated what I entered in the test form, for another test the field was left blank with no history). Where can I see how the columns map to the form questions so that I can troubleshoot? What might cause data to show up for some submissions but not for others?

Try deleting all the empty rows, and then it should populate as expected.
