Smartsheet fillable field

I am unable to get my fillable field to populate the data from my Smartsheet. I have a form that includes logic. While some information from the form submission is successfully transferred to the fillable fields in my PDF , certain sections are not populating in my fillable form. I have included screenshots for reference. For whatever reason, in the material section of the form, it only pulls in the QTY for one line and nothing else. My trigger for the fillable field is a column in my sheet that auto populates once an entry is made on the line. I have switched the trigger to many other things to try to get it to work and the fillable form still populates the same every time. Is it possible that the logic I have set in the form is preventing the information from populating in the fillable fields?

What does a full form entry look like? Can you try generating the doc manually? If so does that work? Is the mapping correct from your Fillable PDF?
