How do I populate time lapse from a date enter in another column and have it caluculated in mo & yrs

I am trying to make a column that automatically calculates a time lapse based on a date entered in a different column. I would prefer it to display in months and years if possible.

Dollar amounts in a form
I am trying to require a form field to only populate a number to display in a dollar format. I can only see the requirement of a number, but not an actual dollar amount. Is this possible in a form?
Copied Smarsheet as new, now Conditional Formatting will not work at all
I copied a Smartsheet and changed some of the column headers. I deleted all the Conditional Formatting because it no longer applied. Then when I set up new Conditional Formatting, very simple - if this cell says YES, make the row green - it will not work at all.
