emailing a form once a checkbox has been activated

Hi Donny,
You might try creating a customized notification email that contains the link to your form. More information on notifications can be found in Create Notifications to Stay on Top of Sheet Changes.
Your notification can send people in a contact column of your sheet an email containing the form link when the checkbox is checked.
One thing to note: currently, notifications can only be sent to people shared to the sheet. In our August release, we plan on adding a feature to allow anyone with an email address to receive a notification.

I would do this using Zapier (www.zapier.com), especially if your survey form is not in Smartsheet.
