Issues with Approval Workflow not following Order of Approvers

Smartsheet is not following the approval workflow I have set up as it previously did. I am now seeing occurances where the an approver(2) and the approver in the following step (3) are sent the approval request at the same time instead of in sequential order based on the first approver's response. The cell history shows both were submitted at the same time following an initial approver's (1) response being recorded. The last approver (3) also sometimes approves first which moves the request along and the (2) approver's response isn't recorded due to the email link being used already. How do I fix this issue?

It sounds like there may be something interfering with the trigger for those approvals. In your automation, are these three approvals separate workflows? What are the triggers for each?
