Changing row information without adding a new row

I have a calendar that is populated by a Form submission. The information captured is an on-call schedule for our department doctors. Often times, the docs will end up switching on-call days/weeks. Is there a way to update the row they already have submitted (deleting/adding dates) instead of creating a new row?

You could use an Update Request, another form, or the Premium App, Dynamic View.

Is there a template for an update request? Or could you give me insights on what template to use to do this?

Update request is a notification that goes out from SMAR, you can set it up in automations OR you can manually request one from the row itself

You can pick who it goes to, schedule when you want it to go out, you can change the text of your email sub and body as well as define what fields you want updates on.
