Conditional Drop-down list

Currently, my company is using smartsheet to build up a database of our quality issues. Since the department is new to this, data entered is not very robust: Many auditors simply log in, type in their name, work station and enter text describing issues as they encounter them.
I want to take this to the next level and create a structural data gathering process, and conditions need to be defined so that issues can be well categorized.
For example, we make this flow (for a phone manufacturer), each step will reveal its own sub-category:
1. Where does the issue occur?
- Workstation 1
- Workstation 2
- Warehouse
- Shipping
User selected Work Station 2
2. What is the nature of the the issue
- Wrong component built
- Missing component
- Missing lables
User selected Wrong component built
3. Which component was incorrect?
- Screen
- Body
- Memory
User selected one and then add additional details if needed.
As you can see, this nested, procedural form would help classify issues in a meaningful way; in this manner, we can find out where the issues occur, the nature of them or let's say what components tend to cause the most issues.
I don't think this can be done in smartsheet - but can it be done using a third party and then integrated into an existing sheet? We like this platform because we can use it for different department and the forms are helpful to share accessibility across the company - but this level of complexity may need some further development.

You could probably do it with Google forms, and then use Smartsheet sync to pass the data through
You can achieve the hierarchy of questions by using sections in Google Forms -
I would suggest using a Google Form to accomplish your needs/desires.

Google forms can handle the branching questions and then you can have your results sent to Smartsheet.
