Pull attachment from Sheet

Is it possible to create a form that pulls attachments and sends back to the requester?

e.g., I have a sheet completely filled with a column titled "Common Number Criteria", and an attachment that corresponds to each number in the column. If I created a form and send that link url to an outside user, can they type the number, submit the form, and the attached file gets emailed to them?

 

Or, does anyone know of the shared apps that have this capability?

There currently isn't a way to automate the act of sending someone a specific attachment based on what they request. If you have development resources available at your organization, you might consider using the Smartsheet API to automate this: https://smartsheet-platform.github.io/api-docs/
