IF(And formulas to count late dates

I would like to create a formula that measures the health of a project (Red, yellow, green) with missing dates:

Expected Date (June 6, 2019)- I assign that date to the vendor

Scheduled Date (this is expected within 5 days of assigned expected date)- this is filled in by vendor

Completed Date (this is expected within 5 days of scheduled expected date)- this is also filled in by vendor

What I would like to have the health colors change when the vendor hasn't placed the dates in to call out the health of that task (also highlighting that it's late).

I am assuming that I would have to have a few health titles; ie. Beginning Project Health, Completing Project Health, etc.
