Is there a work around? Alert or Update Request?
I'll keep this simple.
In the method I'm using the form every person that uses the form is not required to make a Smartsheet account. They have too many logins and adding another "software" to the mix is not going to go over well; however, they are fine using the form.
This means I'm not able to take advantage of the modified by or created by fields.
I do have them selecting their name in a drop down for a Submitted By field and I have a VLOOKUP populating a Follow Up Email cell; however, I just cannot figure out how to use the email address in an automation.
Any suggestions?
I want to have a working automation off the follow up date logic that emails the csr when the follow up date is met, but I can not find a way to do this.

The email address would need to go into a Contact type column. Once you set up your workflow to trigger on the date, you can specify who to send it to by selecting the option of "Contacts in a cell" then selecting the Contact column.

Sadly, that column cannot have formulas, tried that one earlier.
The tracking system can be submitted by anyone, and I use a VLOOKUP to snag the first name and then pull the email field over.

Duh. Sorry about that. It's been a long week.
Would it be feasible to have them manually enter their email address? THAT would allow the Contact column to be used on the form.

I could have them do that, I was thinking about efficiency.. but what's a few more seconds in a report

Unfortunately, that's about the only way to make things work the way you need them to for the Automations.
