Forms - SmartSheet Form not the same as Form received by Action Parties to enter data

Options
  1. I have a Smartsheet that tracks Action Items.
  2. I created a Form that shows Action Parties data from Smartsheet and requires them to enter responses.
  3. The Form sent to Action Parties that appears in Emails and can be opened from the Email to enter responses does not contain the same cells as the Form in SmartSheet. Why?
  4. See attached Word file with clips of the Form. Note I can't see my Attachments when using the Paper Clip so am not sure if it is attached?
