Is there a way to add logic to a statement in a form (not at field)?

I am trying to use Form Logic to display a statement that displays only if "Yes" is selected on a form field, without it being a form field that needs to be completed.

For example, if "Yes" is selected, then I want a statement to display that says "Please contact your manager to discuss further".

Is there any way to do this with Logic? When I try just creating a description, there is no option for logic.

I’d recommend creating another column with the information/message or add a default value to a column.
