Set a link for each set of answers

Hi everyone,

I want to make a form with 4 questions, if the user answered Yes for all questions, a link will appear for them, if they're answering differently, another link will appear for them, and so on. Is it possible to do that in Smartsheet? and how can I include a link in the fields in the form. The image attached will help you to understand my question.

Also, please see the following image:

How can I remove the triangular fillable box under the link, I don't want it to appear in my form.

Yes. You can use the Conditional logic in the form and reveal a field with the link depending on the answers, but unfortunately, you can't remove the field underneath.
