It is easy enough to create a date field on a form. Is it possible to add a date and time field? Obviously, I could use free text but that is not as clean.
The only way to avoid a free text would be to set up dropdown columns with options for hours, minutes, and am/pm.
If you are trying to capture the submission date/time, then you can use a system generated Created (date) column in the sheet.
