Workflow triggers wrong notices

Hi, I have no idea why smartsheet workflow suddenly start working wrongly.

The workflow is to be triggered when a row is added or changed (this usually happens via form which gets submitted). The email address specified is to get a notice and details from selected smartsheet columns but approx one or two weeks ago, the notice is sent out also for some older rows where no change happens.

Can anyone advise what the issue could be? Happy to provide more info.
