What is the best way to show an extension has occurred to the original due date and track changes?

I created a smartsheet to track the requests my organization receives. A response must be provided to each request by a certain date (due date). However, occasionally this due date must be extended. I would like the smartsheet to show:

1. The due date has been extended
2. The new due date
3. The prior due date(s)
4. How many times the due date has been extended

Suggestions on the best way to setup is greatly appreciated!
