Can anyone help me with a sheet that has both forms and workflows included?

I am trying to create a sheet that my manufacturing plants can use to submit service issues and notifies the servicer, then the servicer can provide their solution and close the loop. Below is my desired functionality of the sheet.

I need to utilize forms, because I do not want the vendor to be able to access the sheet and see what issues we are having with other vendors. I would like the workflow to alert the vendor contact email that the plant selects but I can not get it to work. Please help!

Which Smartsheet plan do you have?

Can you describe your process in more detail and maybe share the sheet(s)/copies of the sheet(s) or some screenshots? (Delete/replace any confidential/sensitive information before sharing) That would make it easier to help. (share too, andree@workbold.com)

I would like to know if this has been resolved? I also need to create a form which will trigger request for an approval from multiple approvers. Anyone who can share link on the step-by-step process? Thank you.
