Can a custom header and footer be added to a smartsheet that is permanent?
I see an option to type in a header when the document is printed but that does not become a permanent part of the document that anyone else can see.

You're talking about Document Builder, right? You could create the form-fillable PDF and make a field at the top which you map to a column on your sheet that has what you want the header to be and then drag that into position when editing your document and mapping columns in Document Builder.
