Using Documents in the Form as part of the Form Build

In creating forms, have the ability to embed documents in a form. For example, a user needs to submit an IT ticket and provide the computer name to complete the request. Some users do not know how to locate the computer name. Attaching the instructions (Word Document) will provide the user the steps to locate the name and complete the request.
