Automation sending 2 emails instead of 1 and now its sending 4

I have an automation for a level 2 and 3 release. when level 2 occurs it must first send an approve/decline to the admin director and then once they approve it goes to the medical director but every time the medical director gets 2 emails with a slightly different note message but all the rest is the same. On top of that i tried to separate the requirements for it to send the email and now its sending 4. please help!!!

You can test the automation and have it send you the notification instead. What happens when you do that? Do you get 2-4 notifications? Can we see an example of the notification and can we see the setup of the automation?
