Security/Risk Assessment Form

I'm looking to create a single form or multiple forms that link to each other, if needed. I'm trying to have a Security/Risk Assessment form that I can send to acquired companies to begin filling out as part of integration efforts which will require attachments. Right now I only see one way to upload files and that is once per form. I have a lot of questions that will ask for multiple attachments, ex. Network/Firewall Diagrams, SOPs, PEN Tests, things like that. Looking to make this is a painless as possible for the newly acquired company. Progress tracking would also be a bonus. Thoughts, ideas/suggestions? I'm somewhat new to Smartsheet.

Can you provide me with more specific requirements?

To begin filling out the form, send the update request form to acquired companies. When they update the in the respective fields, the sheet automatically updates.

I was looking to be able to have multiple attachment related questions (form elements), but it only appears you can have 1 per form which makes it difficult to track, if you have, say 20+ questions that all require different attachments.

Unfortunately, it's not possible now, but it's an excellent idea!

Please submit this as a Product Feedback or Idea (If it hasn't been added already) when you have a moment.

Here's a possible workaround or workarounds

• 3rd-party form integration/service
• Another option could be to have a chain of forms for each question, and then they could add an attachment for each.

Is that an option?

Thanks everyone. I think I'll go down the path of a Sheet only and not use a form.
