Form Columns are not appearing in the correct order

I have created a form, but htere are extra columns populating, and some columns are appearing in the wrong order.

Below are the first few fields in my form, but on the spreadsheet there are three extra forms at the beginning.

Also, for example, the Dependent Sex fields are listed with other infomration fields about that Dependent, but the spreadsheet is listing many clumped together after the dependent information section.

Any insight as to why this is happening and how to resolve it would be greatly appreciated.

You can click and drag columns in the sheet to reorder them, and you can click and drag fields in the form to reorder them there.
