Do I need to create multiple sheets for the same form?
So, I created a sales intake form and created a QR code. This works perfectly.
What I would like to do is use the same form for multiple salespeople (7) so that I can track activity.
I am assuming since the QR code is linked to the URL and that data is being dumped into a specific sheet I would need to create a total of (7) copies and just rename the sheet, then merge them into a master sheet.
I am concerned about potential duping contacts and since I have every new lead tied to a custom prospect id (xyz00001) having the proper logic tied so that each leads prospect id is in consecutive order.
Does anyone have any creative ways around this or am I stuck with creating 7 copies of the same form to different sheet names and having the prospect ids unique to the salesperson, then creating the logic to delete dups by prospect name and by created time/date.

Is the issue of tracking which salesperson made the entry? If so, you can set the form so that they have to be logged in, then it will track which email address made each submission. Will that work?
Otherwise, this sounds like a good application for using Appsheet for the form and you should be able to solve your challenges pretty easily. I'm actually working on a template app for people to copy and play around with Appsheet and Smartsheet. Do you happen to be using Google Workspace, by the way?

You could use one form but combine it with the URL-Query feature and have a unique QR code for each salesperson.

How would you work the back end so that the contact manager's name would flow through to the column row?
Example:
Use the URL Query, above once the form is completed the data from the form comes through and the contact manager name gets dumped in the column/row of the lead.
If you could assist me with that piece, I think you might have solved my problem! Thank you!

I just need to make sure that the contact manager's name associated with the URL gets populated in the correct column.
For example, would this URL query automatically populate in the "Contact Manager" column with "Test Manager" in the same row or do I have to add to the string?

I was able to figure it out. I forgot that I needed to include the "Contact Manager" Field in the Form then hide it.
All data was carried over. Sorry for the confusion. Thank you for the assist! :-)
