logic to a form to repeat and answer questions again.
Is there a way to apply logic to a form that when asked at the end if they have another record to input that they can fill out the questions again but some fields do not need to be completed again such as name?

I developed a solution using two forms (or the same form by using conditional logic) that can be used to submit the main information and then the other information on a new row for each submission after the first.

I have this same issue and would love to be able to loop a form for our faculty to provide info. If they have to completely restart the form and enter their name and department every single time, they will not complete the form. I would rather have them input entries, then select if they have another entry and be able to input more information without completely starting over.
