Multiple forms linking to one sheet but Identifying the form name on submission

Hello Smartsheet Community,

I am hoping you can help me with the below.

I have created a sheet which logs all of our machine maintenance checks. On this sheet, I then have multiple forms relating to different machines within our workshop.

Our staff have a portal which shows the individual machine maintenance forms to complete in the dashboard/portal format.

However, when submitted to our MASTER MAINTENANCE sheet, we cannot view what machine has been checked, via the form submitted.

Is there a way that when submitting the form, it logs the name/ title of the form so we can see what machine has been checked?

I hope this makes sense and that someone can help me with this!

I hope you're well and safe!

You can set a default value for a field on the form (which can be hidden) or add a so-called helper column and do the same for each form.
