Smartsheet adding a digit to a numeric field

Just wondering if anyone has run into this. We have providers fill out our forms. A couple of fields are their business Tax ID and/or their individual NPI. We've seen quite a few come in where the number is 1 number off. Of course, the submitter could have made an error but we've seen enough to question if something can be happening.

When you say "off", what exactly do you mean?

I mean if a tax id is 12345688 it comes across as 12345689. Just numerically adds a digit to the last number. If if the last 2 numbers are 89, it'll change it to 90.
