Reports Not Generating Data from One Sheet Only

We have a report set up with the following column headers:
The parameters of the report are set to include a specific sheet from each workspace that includes basically these same columns and that a row should display on this report IF there is not a Jira # (AZZLY Internal) number entered on the source sheet. This works for all of the specified sheets in all workspaces except one, and I have confirmed the same column names are present. Any ideas?

Here is the example of the form that is NOT populating - some columns are different, but the one we are looking for is IF there is a Form Name AND there is NOT a Jira #, then it should display on the report.
We rely on this report to track submissions from clients - any ideas? Thanks in advance.

This is an odd one. When you go to the filters on the report do you see the column name for Form Name or Jira # listed twice as an option for filtering? That may indicate that there is some small difference (if not Column name, perhaps Column type) between the two.
