How do I add multiple check boxes in one column?

I am creating a form from my Smartsheet. I want to have multiple options with check boxes but have not been able to find out to add more than one check box.

Hey @williamsons2,

In the form you need to make the field a dropdown rather than a checkbox (see my previous comment), then toggle multiple values per cell:

Then in the right hand side (when you have the field selected), click either the "Vertical Checkboxes" or "Horizontal Checkboxes" under "Display As":

You'll want to use the "Dropdown List" Column Type, and then select "Allow multiple values per cell".

How do I get it like that on the form? When I choose the checkbox option, it's a single box and doesn't allow me to label it.
