Can I attach a form to a Gantt chart to be sent out automatically when milestone is met?

I want to attach a form to a Gannt Chart, so that when a specific milestone is met, the form will automatically be sent to be filled out. I am using the Gantt chart to track multiple jobs, and I want it to send out checklists at certain times during the job process. I created one of the checklists as a form in another sheet, but I want to know if it is possible to link them so they send automatically? Also, I am using the lowest subscription right now - if it is possible, do I have to upgrade for this ability? Thanks!

    So sheet 1 is where you are looking at the Gantt Chart view, and Sheet 2 is the form. In the grid portion of your Gantt or in Grid View, the duration column would show a 0 when it is a milestone (You have to go and manually change them from 1 day).

    You would then set up an automation on Sheet 1, when a date is reached in your End Date column, with the condition that Duration = 0, then in the message that you are allowed to write, you would enter your form URL with whatever verbiage you want to send out. It can either be sent to a specific person or a person in a contact column (Depending on if it goes to different people at each milestone or not). You can choose to send the info from the row in its entirety, specific columns that you choose, or just the message.

    In the message you are writing, if you want to send the message only, but reference info from the row of the milestone, use {{Column Name}} to insert info. Something like:

    Hello! Your milestone for {{Primary Column Task Name}} is on {{End Date}}. Please fill out the form for an update at: URL.
