Add an active link or document to a form

Hi All -
I can't seem to find a good process for this: add either a document as an attachment, or an active link, to a form that will be sent out to users (sharepoint, via teams). They would open the document or link, then respond to the questions/etc in the form. The users would not be uploading their own documents.
I have tried to add the link to a form field but the link is not active (and it's long).
Goal: a document/link sent with the form, from the administrator to the user. User will access the form/link and fill out the form. Users are internal, does not need to work for outside users.

You can format description text to have a hyperlink behind it 🙂
You can do this either over the full URL, or use regular text:

Hi Darren - yes, I've tried that but it's not a clickable link. It's the full URL (which is long, as sharepoint links tend to be), that the user would need to select, paste and go. While possible, these are busy folks who are not technologically inclined and I want to minimize work (and percived work) for them.
