Use form to send email to take action

Hello, I have a workflow where I am trying to add a email note as a form and asking them to review attached document, take action. For some reason, the automation that I added, does not send the email verbiage I added. What am I doing wrong? attachment added. Please advise.

    Dear Andree, Thank you! I have shared my sheet with you. I have a list of people I need to send email to, once they have been oriented, to complete the COI form. I would like smartsheet to send an email, once the orientation date is applied. The email should include the verbiage I have added tot he automation, the attachment. They should then be able to complete the form, then attach it back to the sheet. I would like to be notified when they attach the sheet back. does this explain?
