Automation Workflow broken?

Has anyone experiencing any "Request for Approval" workflow issue? e.g. approval request goes though a workflow that is NOT as defined - the COO is receiving purchase request that does NOT required his approval. Any help is appreciated.

Here is the snap shots of one of the workflows.

Trigger-> When Condition is Met -> Request an Approval to Requester's manager -> If Approved -> Request an Approval to CTO -> If Approved -> Request Approval to COO (for purchases greater than $10,001)

The strange thing is, COO has been receiving Request an Approval for requests that do not required his signature.

Can you share screenshots of all the workflows or the sheet? I need to see it all to see why it's triggering the way it does.
