Hello.

Is there a way to utilize both automation and forms?

ideally, I want to create a workflow which is triggered weekly or whenever certain changes have been made. I would then want the workflow to sent out a created form to certain people.

Is this possible, or is there another way to accomplish my goal?

Yes, you can add the form link to the Workflow.
