Recurring form submission error

We continue to see this error on forms connected to a specific sheet, even with all inputs given, required or not:

The information you entered is incomplete or invalid.

Once the error has occurred on a form, it persists until "fixed". No one can submit new entries with that form, neither external users nor users in our account. For all intents and purposes, the form appears to be "broken".

The only "fix" we can find is to delete and recreate the faulty form. Sometimes the error occurs from the first submission attempt, and we recreate again.

We have 4 identical forms, except a hidden field with a unique default value in each form. They are all duplicates of the same unused "template" form.

The error has occurred in several of these forms since we created them six months ago. Quite frequently at first, not as frequently since.

The target sheet has another 2 forms, which are not duplicates of the same template form. We have never heard of the error in these.

Are we missing something obvious, or has anyone seen anything similar? I am happy to provide more details 🙂

Form characteristics

  • The forms have two hidden fields with default values. One is a Text/Number column with a unique value in each form. The other is a Dropdown (Single Select) column with the same selection in each form.
  • There is no form logic.

Sheet characteristics

  • 55 columns
  • 6,500 rows
  • 8 automations: 7 notifications, 1 "Copy a row"

    I've figured it out.

    You had a Validation set on the hidden field for the Supplier. Once I removed it, it worked.

    Did that work in the original one as well?

    What happens if you make a copy of the sheet? Does the form work in the copied sheet?

    I'd be happy to take a quick look.

    Can you describe your process in more detail and maybe share the sheet(s)/copies of the sheet(s) or some more screenshots? (Delete/replace any confidential/sensitive information before sharing) That would make it easier to help. (share too, [email protected])

    Thanks for your quick reply.

    I have shared with you a copy of the sheet along with the forms.

    As for the process, the sheet is our registry of non-conformities and deviation requests for manufacturing and operation of machinery.

    We allow some suppliers to submit non-conformities and deviation requests directly to the system. This is what the forms in question are for. One form for one supplier.
