New to smartsheet and created a form to submit a survey; however, in the sheet page the headers are not my custom label which were created for the form. How do I view the actual label text on the sheet to correspond to the question I asked?

I entered some test entries through the "Forms" function and now want to go live with the sheet. It is not allowing me to delete the test entries. When I delete them, they come back when I refresh.
