Why isn't my attachment on the automated email?



I am using a smart sheet auto system where it makes the PDF automatically when someone fills out a form. Once the PDF is made I have a new one to send out an email. see below.

Once they approve it is supposed to send an email with the attachment. but when i get the email back it doesn't have the attachment. Am i doing something wrong in the automation? see below for what the email looks like when i get it back. I just need the attachment to be sent out with the email.
