Wrong Automation is Triggering

Hi all, today is our launch date! We are very excited.
We have approval workflows in place for each manager based on the particular setup request. We also have separate approval workflows in place for client Terminations.
Our very first request received was for a setup - but instead of the setup approval workflow, the termination workflow was triggered. I can't figure out why!!!!

@ktcran could you shortly clarify the trigger is by a form? Then is would be row are added (note changed) with cell value product change … it seems there is no criterie selected . I suggest to modify and to run manual in a test sheet.
