forms setup option, retain the last entry

a new dropdown option for the form setup, selection makes the form reload with the last entiries populated - currently either blank form, or success message (not useful) or send to a link (also not useful).

the example is a punchlist , where e.g. 3 issues (1/ cable not hooked up, 2/ touchup paint needed, 3/ housekeepin) need to be reported for the same part. Currently all the parts info (~3-4drop down menues) needs to submitted every time for the 3 entries. This makes smartsheet useless for the intended application .
