JIRA not triggering connection workflows

We are experiencing issues where changes in JIRA are not triggering the connection tool's workflows. When I look at the log, there are no errors. However, it says "Filter Row Finder" instead of "Sync To Smartsheet". When I manually force the workflows to run, they push incorrect changes back to JIRA instead of being updated by JIRA.

 

Has anyone else had this issue before?
