approval request rule

I tried to run two rules of approval request, the first rule is "When a row is added or changed", the second is "When a row is added", the rest of the two requests are same. 

why the 2nd rule does not work?

Another question is all trigger conditions are jointed by "or", can joint them by "and"?

Can you describe your process in more detail and maybe share the sheet(s) or some screenshots? (Delete/replace any confidential/sensitive information before sharing) That would make it easier to help. (share too, andree@getdone.se)

Question 2.

When you set up the rule, you can add conditions instead and then you can select OR / AND but in the Trigger section it can only be an OR.

Would that work?
